Completed forms can be saved to your computer and emailed to email@example.com or returned to the center. Please fill out contracts completely and return with payment the Wednesday prior to when child care is needed. Your contract is your bill. You can make your payment online, or you can make your payment at the center via cash, check, or credit card. If you need help, please contact the office. Health appraisal forms will be due 30 days after your child's start date. These must be completed and signed by a physician. These forms must be updated yearly.
Parent handbooks are included with your enrollment packet and should be kept for reference. Should you misplace your handbook, you may download/view a copy of it here. Updated December, 2020.
CCDC shirts are what we wear in parades and are required for all field trips. School age children get one free t-shirt with their paid registration. Payment must accompany the form. Adult shirts are gray and youth shirts are green. Adult shirts are not required, but encouraged. We also have cinch sacks and branded child masks! If you have any questions, please contact the office.
If your child has an allergy or a medical condition, these forms must be filled out by your child's primary care physician.
Please download and fill out completely if you would like to volunteer in the center or attend field trips with CCDC. These are required for anyone wishing to volunteer.

With the spread of COVID-19, new health and safety protocols are necessary to ensure children, families, and staff members are as safe as possible. If you are giving or applying any medication to a child in care, the following must be completed by the parent for each medication. An interruption in medication will require a new permission form.
Students attending CCDC during the virtual learning time will be allowed to bring their own computer/tablet. Students and parents involved in bringing a personal electronic device to CCDC must adhere to the Student Personal Technology Contract, as well as all other policies listed
in the center handbook. Please print, sign, and return this document if your child plans on bringing a personal electronic learning device to CCDC during distance learning.
