Completed forms can be saved to your computer (or screen shot if completing by phone) and emailed to ccdcrocks@yahoo.com or returned to the center. Please fill out contracts completely and return with payment the Wednesday prior to when child care is needed. Your contract is your bill. You can make your payment online, or you can make your payment at the center via cash, check, or credit card. If you need help, please contact the office. L  HEALTH APPRAISAL FORM - TODDLER, PRESCHOOL, DAYCARE

Health appraisal forms will be due 30 days after your child's start date. These must be completed and signed by a physician. *These forms must be updated yearly.

 

L APPAREL ORDER FORM

CCDC shirts are what we wear in parades and are required for all field trips. School age children get one free t-shirt with their paid registration. Payment must accompany the form. Adult shirts are gray and youth shirts are green. Adult shirts are not required, but encouraged.

 

L BACKGROUND CHECK FORM

Please download and fill out completely if you would like to volunteer in the center or attend field trips with CCDC. These are required for anyone wishing to volunteer.

 

L SCHOOL AGE HEALTH AGREEMENT FORM

Please complete this form each year for your school age child if you did not do so at registration (editable pdf, or print and return). This form is not for preschool age children.

L BRIGHTWHEEL APP INFORMATION

We use Brightwheel, a tool for classroom management, communication, photos, videos, clock in/out features, and much more. Brightwheel is the industry leader in early education, proven to save time for staff, allowing for measurably more time with students, while also delivering a much better experience for parents. Please create your profile and have the app downloaded before your child begins care at CCDC. Click the above link to download more information including links to instructional "how to" videos.

L MEDICATION PERMISSION AND INSTRUCTIONS SLIP

If you are giving or applying any medication to a child in care, the following must be completed by the parent for each medication. An interruption in medication will require a new permission form.

L DIETARY FORM​
This form must be completed by a licensed physician, physician assistant, or an advanced practice registered nurse, such
as a certified nurse practitioner. Updates to this form are required only when a participant’s needs change.

​

L PARENT ADVISORY COMMITTEE

Involving families in our program is a top priority. This helps children, parents, and our staff make the strong connection between home and center. We are an extension of the family, developing a partnership where sharing support, ideas, and genuine love for children is of the utmost importance. We encourage parents to visit and participate in school activities.
